PlanetHS Athletic Pre-Registration
Dear Parents/Guardians,
The Athletic Department at PHS now hosts our pre-participation athletic forms online with PlanetHS. This will include athlete handbook/drug test consent, proof of insurance, and uploading a picture or scan of your physical (front and back) for approval. This digital platform will allow you and your student(s), to complete and access athletic forms via computer, tablet, or mobile phone. It is HIPPA, COPPA, and FERPA compliant. Parents and student must create separate accounts, using different emails and/or mobile numbers.
